Registration Form

If you would like to enroll your child in the Kindergarten through Eighth Grade Program, click to download a Registration Form (two pages.) If you would like to enroll your child in the All-Day Kindergarten Program, click to download an Afternoon Kindergarten Registration Form, along with the two-page Registration Form. You will need to download a Photo Release Form for all registration forms.

If your child will need medication during school hours, click to download a Medication Permission Form.

Once you have the necessary forms, please fill them out and send them to the school office.
